# Form Settings

The Form Settings admin option provides database details for saving the data of forms.

* Click the ***Form Settings*** option from the admin options panel.

* The ***Form Settings*** page opens.
* Enter the database details to save the form data as mentioned below:
  * ***Usernam***e: Use read-only credentials
  * ***Password***: Use a valid password
  * ***Host***: Provide Host address
  * ***Port***: Provide the port number
  * ***Database Name***: Provide a database name
* Click the ***Save*** option to save the entered settings information.
